• Source of the paid amounts: HHS/CMS Medicaid Provider Spending by HCPCS (T-MSIS). Paid is not the allowed amount, not a fee-schedule rate and not a cash price. HHS suppresses cells below 12 claim lines or 12 patients, and managed-care payer rates are excluded, so small states are incomplete.
